<aside> 💡 In the table below, add 1-4 goals for the next 12 weeks. Remember that each goal should be realistic but challenging and should align with your long-term vision. Also, keep them simple as you will break them down more in the next step.

<aside> 💡 Replace this block with a vision of your life 5-15 years in the future. Try to cover these key areas:
<aside> 💡 This is your weekly planner. In each week, add your actions for that week. These can be things you’ll need to do every week, or just once. For each action, select which Goal they relate to and then how many times you need to complete them each week.
For example, if you need to get a gym membership, that would just go in one week and you enter “1” in the “Times per week” column. Equally, if you want to not drink alcohol at all in this 12 Week Year, then enter that action every week with a “Times per week” of 7.
Try to fill out all 12 weeks at the beginning so you have a complete plan. You can always add or modify actions as you go if needed. Once you’ve done this, scroll down to the final section where you’ll answer some questions to get you motivated and see how to use this planner on a daily basis.

<aside> 💡 Below are some questions that are designed to help you prepare for any challenges you might have when completing this plan. Answer each question to consider the potential obstacles and solutions in your plan and refer back to them as needed.

<aside> 💡 That’s it, you’re all set up! To use the planner each week simply shrink/compress all other weeks so that you just have your weekly actions to look at.
At the start of each day remind yourself of your vision and check your goals for the day. Tick each off as you complete them and keep an eye on your total weekly score at the bottom of the “Score” column.
Remember to try to stay above 85% if possible!
